1. How to create an eForm
Open the Admin Console, select ‘eForms’, and then select ’Forms’ from the expanded section and then click on ‘Create Form’ on top right.
Step 1 - Create the form setup detail
A. Add the 'Title' and 'Short Description'.
B. 'Email 1' and 'Email 2' - Select administrators to receive email copies of the form submission.
Step 2 - Add your questions
A. Click on 'Add Field' to choose the fields.
B. Fill the following details on the created row:
a. Add the 'Field name' that needs to appear on the eForm. This will be the question the parent or carer will see when filling out the form.
b. Select the most appropriate 'Field Type' from the drop-down box. Field types can be required or optional, to make the field 'Required' click on the checkbox next to the 'Field Type'.
C. After entering all the required fields on the eForm, scroll down and click on ‘Save’.

2. How to publish an eForm on the app
Select ‘Quick Compose’, enter your content, select ‘Show Advanced Options’ and click on 'eform, video,map or weblink’. Navigate to ‘Attach eForm’ and select your eform from the dropdown menu. Then click on ‘Save’ and send the notification.
3. How to view submissions on the Admin console
Open the Admin Console, select ‘eForms’, and then select ‘Forms’. On the Forms page click the ‘Submissions’ button for the form. On the Submissions page you can view each entry individually by clicking on ‘View’.
4. How to print the eForm submissions as PDF

Print an individual submission
Select ‘eForms’ from the left hand side menu, and then select ‘Forms’ from the expanded section. From here, select ‘Submissions’. Choose the individual form submissions from the list and click ‘Print’.

Print multiple submissions in bulk
On the Submissions page select multiple entries or click ‘All’ checkbox and click on ‘Print Selected’.
5. How to export the eForm submissions to Excel
On the Submission page, select the ‘Export’ button. Select the ‘Form’ name from the dropdown menu and select the date range the submission was made. Click on ‘Export’. This will export an excel file with all the submissions collated together.
